We've not heard of anything similar happening in Designer in VX.2.2 or any other release. A good check for connectivity integrity is DRC-121, this is designed to ensure all nets have appropriate and intended connections intra-sheet, between sheets and through hierarchy.
It is important to remember that annotate type (link) symbols (off-sheet/intra-sheet) do not drive connectivity, the net name does. The exception is hierarchical (pin type) symbols where their name is directly linked to the pin of the parent hierarchical block.

Unfortunately DRC-121 doesn't catch this; maybe it is the way I described it. See SR #3084859191 for an example. DRC-123 and DRC-823 are the best defense we've found so far.

At this point we can only postulate that the operator didn't have that checked when this occurred.
If I understand the test case then DRC-121 reports a rather obtuse warning, DRC- 123 reports single pin net and DRC-823 reports link/net mismatch:

Robert, I see you have different net names. Can you send me a copy of your schematic so I can see how you've set this up?
Well I don't have your test case so I'm guessing based on the description. Also with DRC-121 I have enabled Use Link symbols as Inter-page and intra-page connectors.
